Indications
Visomox D Ophthalmic Solution is used for:
Treatment of ocular infections caused by susceptible microorganisms, Prevention of inflammation & bacterial infection after ocular surgery, Steroid-responsive inflammatory ocular conditions
Adult Dose
Ophthalmic
One or two drops instilled into the conjunctival sac(s), every 4 to 6 hours.
During the initial 24 to 48 hours.
The dosage may be increased to 1 or 2 drops every two hours.
Care should be taken not to discontinue the therapy prematurely.
In the prevention of infection and post-surgical ocular inflammation
Instill 1 drop, 4 times a day, in the eye to be operated on, from 1 day before surgery until 15 days after surgery.
In patients undergoing cataract surgery
On the day of surgery, instill the medication immediately after eye surgery.
In patients undergoing LASIK refractive surgery:
On the day of surgery, instill the medication at least 15 minutes after ocular surgery.
In eye infections caused by susceptible microorganisms:
Instill 1 drop, 4 times a day, for up to 7 days.

Contra Indications
This eye drop is contraindicated in epithelial herpes simplex keratitis (Dendritic keratitis), vaccinia, varicella, and in many other viral diseases of the conjunctiva and cornea, Mycobacterial infection of the eye and fungal diseases of ocular structures and in individuals hypersensitive to any of the components of the medication.
Precautions
For ophthalmic use only
Prolonged use of steroids may result in glaucoma with damage to the optic nerve, defects in visual acuity and fields of vision, and posterior subcapsular cataract formation.
Pregnancy-Lactation
Pregnancy:
There are no adequate and well-controlled studies in pregnant women. Moxifloxacin & Dexamethasone combination eye drops should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus.
Lactation:
Caution is advised when Moxifloxacin & Dexamethasone combination eye drops is given to nursing women.
Interactions
Adverse Effects
Side effects of Dexamethasone + Moxifloxacin eye prep :
Moxifloxacin: Conjunctivitis, decreased visual acuity, dry eye, ocular discomfort, hyperemia, pain, conjunctival irritation, increased lacrimation, keratitis, papillary conjunctivitis and subconjunctival hemorrhage.
Dexamethasone: Visual acuity & field defects, cataract formation, secondary ocular infection following suppression of host response & perforation of the globe.
Mechanism of Action
Dexamethasone is a synthetic glucocorticoid which decreases inflammation by inhibiting the migration of leukocytes and reversal of increased capillary permeability. It suppresses normal immune response.
Moxifloxacin inhibits the topoisomerase II (DNA gyrase) and topoisomerase IV required for bacterial DNA replication, transcription, repair and recombination.
