Indications

Utifos Oral Powder is used for: Uncomplicated urinary tract infections (acute cystitis), Prostatitis, Prophylaxis of urinary-tract infections in transurethral surgical procedures

Adult Dose

Oral Urinary Tract Infection (Acute Cystitis) Acute cystitis caused by E. coli, E. faecalis Women >18 years: 1 sachet (3 g) PO once as a single dose Males >18 years: 1 sachet (3 g) every 2-3 days for 3 doses Multi-drug resistant UTI: 2-3 g PO every 2-3 days for 3 doses Prostatitis 3 g PO every 3 days for 21 days Prophylaxis of urinary-tract infections in transurethral surgical procedures 3 g, PO to be given 3 hours before surgery. Dose may be repeated once, 24 hours after surgery

Renal impairment Oral Fosfomycin CrCl >10 mL/min: No dosage adjustment needed. CrCl <10 mL/min (severe impairment): Use with caution or Avoid; limited data. Hemodialysis (HD): No specific adjustment recommended for a single dose.

Administration

Reconstitution: Oral powd: Dissolve the contents of a single-dose sachet containing 3 g of the drug in 90-120 ml (half cup) of water of water and stir to dissolve. Do not use hot water. This should be taken immediately after dissolving in water. Do not use hot water. Should be taken on an empty stomach. Take on an empty stomach 1 hr before or 2 hr after meals.

Contra Indications

Fosfomycin is contraindicated in patients with known hypersensitivity to the drug and patients with severe renal insufficiency and patients undergoing hemodialysis.

Precautions

Clostridium difficile-associated diarrhea (CDAD) has been reported with the use of nearly all antibacterial agents, including Fosfomycin. Treatment with antibacterial agents alters the normal flora of the colon, leading to overgrowth of C. difficile. Do not use more than a single dose of Fosfomycin to treat a single episode of acute cystitis. Repeated daily doses of Fosfomycin did not improve the clinical success or microbiological eradication rates compared to single-dose therapy, but did increase the incidence of adverse events. Hepatic injury, including hepatitis and steatosis reported; monitor liver function; effect is reversible upon discontinuation of drug Prolonged use may result in bacterial or fungal superinfections, which may appear upt to two months postantibiotic treatment Hypersensitivity reactions, including anaphylactic shock reported; discontinue use at first signs of hypersensitivity reactions and institute supportive measures Monitoring Parameters Monitor serum electrolyte levels and water balance; signs and symptoms of UTI.

Interactions

Additive or synergistic effect w/ beta-lactam antibiotics (e.g. penicillin, ampicillin, cefazolin, carbapenems) and anti-staphylococcal agents (e.g. linezolid, quinupristin/dalfopristin, moxifloxacin). Adverse Effects

Side effects of Fosfomycin Trometamol : 1-10% Anorexia,Diarrhea,Epigastric,Discomfort,Headache,Nausea,Rash,Vomiting <1% Dizziness,Drowsiness,Fatigue,Pruritus Potentially Fatal: Anaphylactic shock, antibiotic-associated colitis and pseudomembranous colitis.

Mechanism of Action

Fosfomycin is a phosphoenolpyruvate (PEP) analogue that irreversibly inhibits enolpyruvate transferase (MurA), which prevents peptidoglycan biosynthesis at an earlier site compared to beta-lactams with no cross-resistance with other agents. Thus, it inhibits bacterial cell wall biogenesis & shows bactericidal effects.
