Indications
Sistin Tablet is used for:
Allergic rhinitis, Hay fever, Allergies, Urticaria
Adult Dose
Oral
Allergic conditions (Seasonal Allergic Rhinitis, Perennial Allergic Rhinitis, Chronic Urticaria), Relief of symptoms from colds, urticaria, angioedema, anaphylactic reactions, pruritus, allergic conjunctivitis, sneezing, rhinorrhea, nasal pruritus, ocular pruritus, tearing, and redness of the eyes.
Adult:
5-10 mg once daily, depending on severity of symptoms; not to exceed 10 mg once daily.
Most patients in clinical trials started at 10 mg.
Syrup:
2 teaspoons once daily.
Injection
IV
Indicated for acute urticaria
10 mg IV once every 24 hours as needed
Child Dose
Oral

Tablet
Children 12 Years and Older: The recommended initial dose of Cetirizine is 5 or 10 mg once daily, depending on symptom severity.
Children 6 to 11 Years: The recommended initial dose of Cetirizine is 5 or 10 mg once daily depending on symptom severity.
Syrup
Children 6 to 11 Years: The recommended initial dose of Cetirizine is 5 or 10 mg (1 or 2 teaspoons) once daily depending on symptom severity.
Children 2 to 5 Years: The recommended initial dose of Cetirizine syrup is 2.5 mg (½ teaspoon) once daily.
Maximum dose of 5 mg per day given as 1 teaspoon (5 mg) once daily, or as ½ teaspoon (2.5 mg) given every 12 hours.
Children 6 months to < 2 years: The recommended dose of Cetirizine syrup in children 6 months to 23 months of age is 2.5 mg (½ teaspoon) once daily.
The dose in children 12 to 23 months of age can be increased to a maximum dose of 5 mg per day, given as ½ teaspoonful (2.5 mg) every 12 hours.
Injection
IV
Indicated for acute urticaria in adults and children aged >6 months
6 months to 5 years: 2.5 mg IV once every 24 hours as needed
6 to <12 years: 5 or 10 mg depending on symptom severity IV once every 24 hours as needed
>12 years: 10 mg IV once every 24 hours as needed
<2 years: Safety and efficacy not established
Renal Dose
Renal impairment
PO
GFR >50 mL/min: Dose adjustment not necessary
GFR <50 mL/min: 5 mg PO qDay
Peritoneal dialysis: 5 mg PO qDay
Intermittent hemodialysis: 5 mg PO qDay; may also administer 3 times weekly
IV
Mild-to-severe and patients on dialysis: No dosage adjustment necessary; monitor for antihistaminic side effects
Administration
ORAL
May be taken with or without food.
IV Administration
Administer as an IV push over 1-2 minutes
Contra Indications
Hypersensitivity to cetirizine hydrochloride or any of its ingredients, levocetirizine, or hydroxyzine
Precautions
Avoid alcohol, sedatives, and tranquilizers, due to increased risk of drowsiness
May cause CNS depression; avoid activities requiring mental alertness until accustomed to medication
Use caution in hepatic and renal impairment
The elderly may be more sensitive to adverse effects
Tasks requiring mental alertness eg, driving or operating heavy machinery
Pregnancy-Lactation
Pregnancy
There are no adequate and well-controlled studies in pregnant women
Lactation
Cetirizine has been reported to be present in human breast milk
In mice and beagle dogs, studies indicated that cetirizine was excreted in milk
When a drug is present in animal milk, it is likely it will be present in human milk
Interactions
Additive CNS depression when administered concomitantly w/ other CNS depressants (e.g. sedatives, tranquilisers).

Mechanism of Action
Cetirizine is a potent and highly selective antagonist of the peripheral histamine H1-receptor on effector cells in the GI tract, blood vessels and resp tract.
