Indications

G Benzathine Penicil Injection is used for: Acute otitis media, Group A Strep. infections, Rheumatic fever, Syphilis, Neurosyphilis, Streptococcal impetigo, Anthrax, Diphtheria, Uncomplicated pneumococcal pneumonia, Streptococcal pharyngitis

Adult Dose

Adult: IM: 0.3 to 1.2 million units up to 2.4 million units as a single dose repeated every five to seven days. Specific dosage schedule: Treatment of group-A beta-hemolytic streptococcal infections: A single intramuscular dose of 1.2 million units. Prevention of rheumatic fever: To prevent recurrence of acute rheumatic fever 1.2 million units may be given intramuscularly every 4 weeks. OR 600,000 units IM every 2 weeks Treatment of acute otitis media: A single injection of 600,000 units. Treatment of syphilis: Early syphilis- 2.4 million units as a single dose. Late syphilis- 2.4 million units weekly for three successive weeks (Total 3 doses). Neurosyphilis- Benzylpenicillin, intravenously, 2 to 4 million units every 4 hours for 10 days, followed by benzathine penicillin intramuscularly, 2.4 million units weekly for three successive weeks. Primary and secondary syphilis have been treated with single injections of 2.4 million units of long-acting benzathine penicillin weekly for 2 to 4 weeks with satisfactory results. Erysipelas, Yaws, Pinta 1.2 million units IM as a single dose.

Child Dose

Child: IM Congenital syphilis: Asymptomatic infants with normal cerebrospinal fluid, intramuscularly 50,000 units per kg in a single dose. For streptococcal infection Children may be given intramuscular doses of 0.3 million to 0.9 million units according to body weight. To prevent recurrence of acute rheumatic fever 1.2 million units may be given intramuscularly every 4 weeks; doses of 0.6 or 1.2 million units have been recommended for children.

Renal Dose

Renal Impairment CrCl 10-50 mL/min: Decrease dose by 25% CrCl <10 mL/min: Decrease dose by 50-70% Hemodialysis: Removed by hemodialysis; administer after dialysis

Administration

IM Administration Do not inject near artery or nerve (may result in permanent neurologic damage) Neonates, infants, small children: Midlateral aspect of thigh preferable Older children and adults: Deep IM injection in upper outer quadrant of buttock Because of high concentration of suspended matter, needle may be blocked if injection is not made at slow, steady rate

Contra Indications

Hypersensitivity to penicillins or cephalosporins. IM inj (ischaemic reactions may occur).

Precautions

Not for IV use Inadvertent intravenous administration, which has been associated with cardiorespiratory arrest and death reported; do not inject drug intravenously or admix with other intravenous solutions For deep IM administration only; do not administer IV, SC, or IT Do not inject near nerve or artery Severe cutaneous adverse reactions (SCAR), such as Stevens-Johnson syndrome (SJS), toxic epidermal necrolysis (TEN), drug reaction with eosinophilia and systemic symptoms (DRESS), and acute generalized exanthematous pustulosis (AGEP) reported in patients taking beta-lactam antibiotics; discontinue therapy when SCAR suspected; consider an alternative treatment Pseudomembranous colitis reported with antibacterial agents, including penicillin G Administer by deep intramuscular injection only in the upper, outer quadrant of buttock (dorso-gluteal) or ventro-gluteal site; quadriceps femoris fibrosis and atrophy reported following repeated intramuscular injections of penicillin preparations into anterolateral thigh and is not recommended Monitoring Parameters Monitor serum potassium concentration, renal, hepatic and hematological status periodically in patients on long-term treatment. Monitor the patient for hypersensitivity reactions for at least 30 minutes after the injection.

Pregnancy-Lactation

Pregnancy Category: B Lactation: Excreted into breast milk, caution advised

Interactions

Probenecid prolongs T1/2 of benzathine benzylpenicillin. Bacteriostatic drugs e.g. chloramphenicol, tetracyclines; other antibacterials; anticoagulants.

Mechanism of Action

Benzathine benzylpenicillin has the same antimicrobial action as benzylpenicillin which exerts its bactericidal action on growing and dividing bacteria by inhibiting bacterial cell wall synthesis. IM admin results in a prolonged effect, but because of its relatively low blood concentrations, its use should be restricted to microorganisms that are highly susceptible to it.
