Indications

Dytor Tablet is used for: Oedema, Hepatic Cirrhosis, Hypertension

Adult Dose

Oral Oedema Indicated for the treatment of oedema associated with heart failure or renal disease 10-20 mg PO once daily initially; may increase the dose by doubling until the desired diuretic effect is achieved Doses >200 mg have not been adequately studied Hepatic Cirrhosis Indicated for the treatment of oedema associated with hepatic disease 5-10 mg PO once daily initially with aldosterone antagonist or potassium-sparing diuretic; may increase the dose by doubling until desired diuretic effect achieved Max: 40 mg daily. Hypertension The recommended initial dose is 5 mg once daily. After 4-6 weeks, increase to 10 mg once daily, if needed. If 10 mg is insufficient, consider adding another agent. Hepatic impairment No dosage adjustments provided in prescribing information Use with caution Contraindicated in hepatic coma

Administration

May be taken with or without food.

Contra Indications

Hypersensitivity to torsemide or to povidone. Hypersensitivity to sulfonylureas, renal failure with anuria, hepatic coma and pre-coma, hypotension, cardiac arrhythmias. Pregnancy and lactation.

Precautions

Patient with difficulty in micturition including prostatic hypertrophy, hyperuricaemia, gout, diabetes mellitus, cardiac arrhytmias (e.g. sino-atrial-block, 2nd or 3rd degree atrioventricular block). Patient with CV disease (particularly those receiving cardiac glycosides); liver cirrhosis and ascites (IV). Patient undergoing surgery; avoid use in the immediate postoperative period after bariatric surgery. Monitor fluid status and renal function to prevent azotemia, oliguria, and reversible increases in blood urea nitrogen (BUN) and creatinine Can cause potentially symptomatic hypokalemia, hyponatremia, hypomagnesemia, hypocalcemia, and hypochloremic alkalosis; therapy can cause an increase in blood glucose levels and hyperglycemia; asymptomatic hyperuricemia can occur, and gout may rarely be precipitated; monitor serum electrolytes and blood glucose periodically Not recommended for the initial treatment of hypertension. Not for use in the treatment of hypertension in patients with Addison's disease. Renal and hepatic impairment. Pregnancy and lactation.

Pregnancy-Lactation

Pregnancy No data are available on use in pregnant females and risk of major birth defects or miscarriage Animal data In pregnant rats and rabbits administered 50x and 6.8x the human dose, decreases in body weight, decreased fetal resorption, and delayed ossification was observed Lactation There are no data regarding presence of torsemide in human milk or effects of on breastfed children Diuretics can suppress lactation

Interactions

Increased risk of severe hypokalaemia with amphotercin B, corticosteroids, carbenoxolone, hypokalaemia-causing medications. Increased risk of lithium toxicity. Increased potential for ototoxicity and nephrotoxicity with nephrotoxic or ototoxic medications e.g. aminoglycosides. High dose salicylates may increase the risk of salicylate toxicity. Increased risk of toxicity with digoxin. Reduced diuretic effect with NSAIDs. Increased risk of hypotension with antihypertensives.

Mechanism of Action

Torasemide, a sulfonylurea loop diuretic, acts from within the lumen of the thick ascending portion of the loop of Henle, where it inhibits the Na+/K+/2CI--carrier system.
