Indications
Carbiroid Tablet is used for:
Grave's disease, Hyperthyroidism, Diabetic nephropathy, Preparation for thyroidectomy
Adult Dose
Oral
Hyperthyroidism, Adjunct to radio-iodine therapy, Grave's disease, Preparation for thyroidectomy
Adult: Initially, 20-60 mg daily as a single or divided dose, reduced gradually once euthyroidism is achieved.
Maintenance: 5-15 mg daily for at least 1 year or 18 months.
Child Dose
Oral
Hyperthyroidism; Adjunct to radio-iodine therapy; Preparation for thyroidectomy
Child: 3-17 yr Initially, 15 mg daily, adjusted according to response.
Neonates & Children below 12 years: The usual initial dose is 250 mcg /Kg/day in divided doses Duration of treatment: 18 to 24 months
Renal Dose
Administration
May be taken with or without food. Take consistently w/ or w/o meals.
Contra Indications
Hyperthyroidism due to nodular goitre.
Precautions
Liver diseases, pregnancy, lactation.
Pregnancy-Lactation
Carbimazole may be given during pregnancy to a thyrotoxic patient, but the smallest effective dose should be used an overdosage adversely affects the foetus. Carbimazole crosses the placenta and is excreted into the breast milk.
Carbimazole may, therefore, cause fetal or neonatal hypothyroidism and goiter.
Interactions
Increased serum levels of theophylline or may develop toxicity. May enhance the effect of anticoagulants.
Carbimazole may interact adversely with other medicines. Iodine or iodine excess may decrease the response to Carbimazole, requiring an increase in dosage or longer duration of therapy with antithyroid agents.
As the thyroid and metabolic status of the patient decreases toward normal, response to oral anticoagulants may decrease, however, if thioamide-induced hypoprothrombinemia occurs, anticoagulant effects may be enhanced. Adjustment of oral anticoagulant dosage on the basis of prothrombin time is recommended.
Serum concentrations of digoxin and digitoxin have been reported to increase as the thyroid and metabolic status of patients taking antithyroid agents decreased,
reduction in dosage of any digitalis glycoside may be necessary as patients become euthyroid.
Potentially Fatal: Increased sensitivity to warfarin in hyperthyroidism and careful control required as the patient is rendered euthyroid.
Adverse Effects
Side effects of Carbimazole :
Mild leucopaenia, sore throat, mouth ulcers, fever, bruising, malaise; rarely cholestatic jaundice; nephrotic syndrome. Nausea, loss of taste, headache, arthralgia, alopoecia.
Potentially Fatal: Agranulocytosis, aplastic anaemia, hypoprothrombinaemia, neutropaenia; hypothyroidism and goitre.
Mechanism of Action
Carbimazole is metabolised to methimazole which is responsible for its antithyroid action. It blocks the production of thyroid hormones by inhibiting thyroid peroxidase.
