Indications
Arocef Powder for Suspension is used for:
Pharyngitis, Susceptible infections, Sore throat, Tonsillitis, Urinary tract infection, Reproductive tract infection, Skin infections
Adult Dose
Oral
Tonsillitis; Skin and skin structure infections; Streptococcal pharyngitis; Urinary tract infections
Adult: 1-2 g daily as a single or in 2 divided doses.
Pharyngitis
1 g PO qDay or divided q12hr for 10 days
Skin & Skin Structure Infections
1 g PO once daily or divided q12hr
Tonsillitis
1 g PO once daily or divided q12hr for 10 days
Urinary Tract Infections
2 g PO once daily or divided q12hr
Endocarditis
Prophylaxis (dental, upper respiratory procedure): 2 g PO 1 hour before the procedure
Child Dose
Oral
Tonsillitis; Skin and skin structure infections; Streptococcal pharyngitis; Urinary tract infections
Child: >6 yr <40 kg: 30-50 mg/kg daily as a single or in 2 divided doses, up to 100 mg/kg daily.
Endocarditis
Prophylaxis (dental, upper resp procedure): 50 mg/kg PO 1 hour before the procedure, no more than 2 g
Susceptible Infections
30 mg/kg/day PO divided q12hr
Streptococcal Pharyngitis
30 mg/kg PO once daily for 10 days; not to exceed 1 g/dose
Renal Dose
Renal Impairment
1 g initially THEN 500 mg maintenance dose at intervals
CrCl 25-50 mL/min: q12hr
CrCl 10-25 mL/min: once daily
CrCl <10 mL/min: q36hr
Administration
May be taken with or without food. May be taken w/ meals to reduce GI discomfort.
Contra Indications
Hypersensitivity to cephalosporins.
Precautions
BEFORE THERAPY WITH CEFADROXIL IS INSTITUTED, CAREFUL INQUIRY SHOULD BE MADE TO DETERMINE WHETHER THE PATIENT HAS HAD PREVIOUS HYPERSENSITIVITY REACTIONS TO CEFADROXIL, CEPHALOSPORINS, PENICILLINS, OR OTHER DRUGS.
Adjust dose in severe renal insufficiency (high doses may cause CNS toxicity); prolonged use may result in superinfection.
Pseudomembranous colitis has been reported with nearly all antibacterial agents, including cefadroxil, and may range from mild to life-threatening. Therefore, it is
important to consider this diagnosis in patients who present with diarrhea subsequent to the administration of antibacterial agents.
Cefadroxil monohydrate should be used with caution in the presence of markedly impaired renal function (creatinine clearance rate of less than 50 mL/min/1.73 M2).
Cefadroxil should be prescribed with caution in individuals with a history of gastrointestinal disease, particularly colitis.

Interactions
Concomitant use w/ bacteriostatic antibiotics (e.g. tetracycline, erythromycin, sulfonamides, chloramphenicol) may cause antagonistic effect.
May potentiate nephrotoxic effects of aminoglycoside antibiotics, polymyxin B, colistin or high-dose loop diuretics.
May enhance the anticoagulant effect of vit K antagonists.
May diminish the therapeutic effect of BCG, typhoid vaccine and Na picosulfate.
May attenuate the effect of oral contraceptives.
Increased serum concentration w/ probenecid. Reduced bioavailability w/ colestyramine.

Adverse Effects
Side effects of Cefadroxil :

Abdominal pain,Dyspepsia,Nausea,Vomiting,Rash,Urticaria,Pseudomembranous colitis,Stevens-Johnson syndrome,Anaphylaxis,Transaminases increased,Neutropenia
Potentially Fatal: Anaphylactic reaction; pseudomembranous colitis.
Mechanism of Action
Cefadroxil binds to one or more of the penicillin-binding proteins (PBPs) which inhibits the final transpeptidation step of peptidoglycan synthesis in bacterial cell wall, thus inhibiting biosynthesis and arresting cell wall assembly resulting in bacterial cell death. Cefadroxil is not active against Proteus, Pseudomonas, Enterobacter, Morganella, Serratia and Listeria monocytogenes.
