Indications

A Clox Powder for Suspension is used for: Staphylococcal infections resistant to benzylpenicillin

Adult Dose

Oral Adult: 250-500 mg 4 times/day. Parenteral Adults: 250 to 500 mg i.m. or i.v. every 6 hours. For more severe infections (e.g. meningitis, endocarditis), 2 g 4 hrly. The maximum dosage for adults is 6 g/day.

Child Dose

Oral Child: 50-100 mg/kg in divided doses every 6 hr. Up to 5 kg (11 lb) body weight: 250 mg/day. Over 5 kg (11 lb) up to approximately 40 kg (85 lb) body weight: 50 mg/kg/day. Total daily dosage must be divided into 4 doses, 1dose given every 6 hours. In infections associated with streptococcus pyogenes, treatment should be continued for at least 10 days to reduce the risk of glomerulonephritis or rheumatic fever. Parenteral Children (up to 20 kg): 25 to 50 mg/kg/day into 4 equal doses administered i.m. or i.v. every 6 hours.

Hypersensitivity to cloxacillin and other penicillins. Concomitant live bacterial vaccines

Precautions

Patient w/ allergies esp beta-lactam allergy, asthma, history of seizure disorder. Renal impairment. Pregnancy and lactation. Prolonged use may result in superinfection Elimination of drug in neonates is slow; use caution Monitoring Parameters Observe for signs and symptoms of anaphylaxis during 1st dose. Monitor CBC w/ differential (prior to initiating therapy and wkly thereafter), periodic BUN, creatinine, hepatic function. Monitor PT if patient is taking warfarin concurrently

Interactions

May diminish the effect of BCG and typhoid vaccine. May increase risk of methotrexate toxicity. May diminish the therapeutic effect of Na picosulfate. May decrease serum concentrations of mycophenolate. May prolong bleeding time w/ anticoagulants. Increased serum concentrations w/ probenecid. Mechanism of Action

Cloxacillin inhibits bacterial cell wall synthesis by binding to 1 or more of the penicillin-binding proteins (PBPs) which in turn inhibit the final transpeptidation step of peptidoglycan synthesis in bacterial cell walls. Bacteria eventually lyse due to ongoing activity of cell wall autolytic enzymes (autolysins and murein hydrolases) while cell wall assembly is arrested.
